Care Management

An emerging concept that refers to a set of activities that are tailored to the individual patient, and that ensure each patient has his or her own coordinated plan of care and services.

Care Plan

 Developed collaboratively by the patient and care providers, a care plan is designed and implemented to optimize the patient’s health status and quality of life. 


 The study of the patterns, causes, and effects of health and disease conditions in defined populations. It aims to identify risk factors for disease and targets for preventive medicine.  It also informs policy decisions.

 Federally Qualified Health Center (FQHC)

 An FQHC is a community health center that gets special funding from the government to provide services to people regardless of their insurance status or ability to pay.

 Health Resources and Services Administration (HRSA)

HRSA is the primary Federal agency for improving access to health care services for people who are uninsured, isolated, or medically vulnerable.

Integrated Health Care

 Care that is informed by these ideas:

·        The mind and the body are connected and should be considered together

·        We all have something to gain from creating a healthier community

·        Accessing health care should be easy

·        We all need community-based supports
