Salary Range $58,367- $79,591 ANNUALLY

General Statement of Duties Under the direction of the Chief Clinical Officer and within established policies and procedures, assists the providers and other clinic team members as needed, supervises support staff. Will serve on an interdisciplinary team which is capable of meeting the physical and mental needs of persons served. As a member of the interdisciplinary team, the RN care manager will participate in selecting strategies to implement evidence based wellness and prevention strategies, participate in initial care plan development, and communicate with medical providers and hospitals. They will also provide education in health conditions, medications, and strategies to implement care plan goals. The RN care manager will be responsible to monitor assessments and screening to assure findings are integrated into the care plan.
Minimum Requirements Graduation from an accredited school of Nursing, licensed as a registered nurse (RN) in the State of Michigan. Proficiency with Electronic Health Records. At least 2 years’ supervisory experience.
Preferences Experience working with vulnerable populations such as individuals who are homeless, have a mental illness and those with addiction disorders. Experience in quality assurance and quality improvement programs and required activities.
Additional Requirements Valid Michigan Driver’s license and use of a vehicle during working hours. Must be able to meet the physical and sensory demands of the position. Must submit a copy of college transcripts and license with application.
