|General Statement of Duties
||Under the direction of the Unit Supervisor, functions as a team member providing professional services in accordance with the person-centered plans of person served, and coordinate the provision of supports and services for adults with serious and persistent mental illness and/or developmental disabilities who need assistance in accessing, coordinating and monitoring resources and services. Assists the individual and their support network to identify, select, obtain and coordinate both paid and unpaid or natural supports to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in their Individualized Plan of Services (IPOS); performs related work as required.
||Bachelor’s degree in Social Work with a Michigan Bachelor’s Level Social Worker (LBSW) credential. Will consider a candidate with a Michigan Limited License Bachelor’s Level Social Worker (LLBSW) or Michigan Limited License Master Level Social Worker (LLMSW) credential or a candidate with a Bachelor’s or Master’s degree in a Human Service field. A candidate with a Bachelor’s or Master’s degree in Human Service field must also possess a Licensed Bachelor Level Social Worker (LBSW) credential or Master Level Social Worker (LMSW) credential.
Will also consider a candidate who is LLBSW or LBSW eligible at the time of hire, must obtain the LLBSW or LBSW within 6 months of hire and prior to the expiration of the initial probationary period of employment.
||LBSW or LMSW AND one (1) year of clinical experience with adults with severe mental illness.
||Must have a valid Michigan Driver’s License, current automobile insurance, and the use of a personal vehicle during work hours. Must be willing to transport persons served using personal or agency vehicles. Must submit copy of college transcripts and license with application if applicable.
