Details for BEHAVIORAL HEALTH CARE COORDINATOR – STATE INNOVATION MODEL (SIM) - 1 Contractual Position Career Opportunity:

Salary Range $23.00/hr. - $27.00/hr.

General Statement of Duties This position is part of a community partnership effort, funded for two years under the State Innovation Model (SIM) grant, focusing on reduction of inappropriate and preventable emergency room utilization. Under the direction of designated Genesee Health System Program/Department Manager, the Behavioral Health Care Coordinator performs responsible tasks in providing behavioral health screening, assessment and coordination supports within a hospital emergency room environment; facilitates the exchange of information between service providers and hospitals regarding clients and programs; provides care coordination between hospitals, primary care physicians, schools, GHS and other behavioral/mental health providers, Probate Court, group homes, Crisis Residential facilities, Crisis Stabilization Team, clinical programs, and the legal system; performs related work as required.
Minimum Requirements Bachelor’s degree in Social Work with a Michigan Bachelor’s Level Social Worker (LBSW) credential, Clinical. OR Bachelor’s in Nursing and licensed as a registered nurse (RN) in the State of Michigan May consider Bachelor’s or Master’s degree in a Human Service field.
Preferences LBSW or LMSW AND one (1) year of clinical experience with adults with severe mental illness
Additional Requirements Must submit copy of college transcripts and license with application if applicable. Must have a valid Michigan driver’s license, current automobile insurance, and the use of a vehicle during work hours. Ability to physically perform the tasks. May be assigned to work evenings and weekends.
